The form comes in two parts. Download them both by right-clicking the link and choosing 'Save Target As'.

Postal Enrolment

Filling them In

1. Complete the enrolment form. USE BLOCK CAPITALS. Where possible name an alternative course. Fee remitted students must write their benefit number or National Insurance Number in the box provided, indicate the benefit they receive and sign.

2. Enclose the fee payment by cheque, card or postal order. Cheques should be made payable to ‘Middlesbrough Council’. You should not enclose cash in envelopes. Students should include their name, address, course name and number on the reverse of their cheque. Remember to include the postcode.

3. Please enclose a stamped addressed envelope for your confirmation and receipt. Applications will not be processed unless an SAE is enclosed. As it may take some time to process your application it is anticipated that your receipt will be posted within two to three weeks*.

4. Your enrolment can be posted to the Southlands Centre.

You can send in postal enrolments any time after 3rd August 2009. Any students who believe they are entitled to fee remission will be required to produce evidence at the first class meeting.

5. Enrolments in person from the 2nd September.
    Monday-Friday 9.30-4.30

Fraud is a Crime

The AE Service participates in the council's anti-fraud investigations and will prosecute people who make claims to reduced fees to which they are not entitled. The loss of fee income is causing the service problems which we are trying to avoid in order to keep the fees down
